What does federal IDR cost?

Separate the government administrative fee, the certified IDR entity’s fee, and Clearest’s service fee when reviewing an engagement. A quoted service percentage does not explain every dispute expense.

FeeWhat it coversWhat to confirm
Government administrative feeCMS lists $15 per party, per dispute initiated on or after June 11, 2026.Use the initiation date and applicable CMS fee schedule; an older dispute may have a different fee.
Certified IDR entity feeA separate fee for the independent organization deciding the dispute.Check the selected entity’s published fee for the dispute type, payment instructions, and applicable allocation or refund rules.
Clearest service feeRecovery-based service pricing, with the percentage discussed during consultation.Confirm the fee basis, billing schedule, expenses, and responsibilities in the agreement and Statement of Work.

Check CMS’s administrative fee timeline, the certified entity fee list, and fee-submission guidance. Ask who advances each expense and when it is reimbursed. The applicable instructions and your written terms control those arrangements.

Does the service fee follow an award or collected cash?

A favorable determination and money received are separate events. Clearest’s published services agreement ties Services Fees to amounts actually received and retained by the client; the Statement of Work sets the fees and payment schedule. Review the agreement’s expense and clawback provisions as well as the service percentage.

  • Which received amounts form the fee base, and how are initial payments distinguished from later recoveries?
  • What happens if the payer pays only part of a determination or a payment is later reversed?
  • Who supplies receipt records, posts payments, and reconciles fees, refunds, and outstanding balances?
  • Which expenses remain payable if a dispute is ineligible, withdrawn, settled, unsuccessful, or unpaid?

Use the fee and refund record example to separate expenses advanced, refunds confirmed, and refunds received.

For a claim-level record, see our payment reconciliation guide. A published pricing summary does not replace the terms agreed for your practice.

Frequently Asked Questions

What does "success-based" mean?

Clearest offers recovery-based pricing. The published services agreement ties Services Fees to amounts actually received and retained by the client. Confirm the percentage, fee basis, payment schedule, and expense arrangements in your Statement of Work.

How long does the process take?

Timing depends on the claim, records, negotiation, eligibility review, and applicable filing requirements. Ask for the next steps for your case. A determination and receipt of payment are separate milestones.

What types of claims do you handle?

Clearest assesses potentially eligible out-of-network payment disputes under the No Surprises Act. The plan, service, payment or denial record, and applicable federal or state route need review. A coverage appeal may require a different process.

How will we share claim records?

Confirm the required records, transfer method, permissions, and operational contacts during onboarding. System connections and supported handoff options should be confirmed for your engagement.
